NM_005359.6(SMAD4):c.*4C>A was classified as Uncertain significance for Hereditary cancer-predisposing syndrome; Familial thoracic aortic aneurysm and aortic dissection by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.*4C>A variant is located in the 3' untranslated region (3&rsquo; UTR) of the SMAD4 gene. This variant results from a C to A substitution the last translated codon. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 85000 alleles tested) in our clinical cohort. This nucleotide position is well conserved in available vertebrate species. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
