Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000535.7(PMS2):c.1088T>G (p.Phe363Cys), citing Ambry Variant Classification Scheme 2023. This variant lies in the PMS2 gene (transcript NM_000535.7) at coding-DNA position 1088, where T is replaced by G; at the protein level this means replaces phenylalanine at residue 363 with cysteine — a missense variant. Submitter rationale: The p.F363C variant (also known as c.1088T>G), located in coding exon 10 of the PMS2 gene, results from a T to G substitution at nucleotide position 1088. The phenylalanine at codon 363 is replaced by cysteine, an amino acid with highly dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 110000 alleles tested) in our clinical cohort. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
