Pathogenic for Cardiac anomalies - developmental delay - facial dysmorphism syndrome — the classification assigned by Variantyx, Inc. to NM_015335.5(MED13L):c.811_814del (p.Val271fs), citing Variantyx Assertion Criteria 2022. This variant lies in the MED13L gene (transcript NM_015335.5) at coding-DNA position 811 through coding-DNA position 814, deleting 4 bases; at the protein level this means shifts the reading frame starting at valine residue 271, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This is a frameshift variant in the MED13L gene (OMIM: 608771). Pathogenic variants in this gene have been associated with autosomal dominant impaired intellectual development and distinctive facial features with or without cardiac defects. This variant likely occurred de novo in the current proband; however, the possibility of parental germline mosaicism cannot be excluded (PS2_Moderate). This variant introduces a premature termination codon in exon 6 out of 31 and is expected to result in loss of function, which is a known disease mechanism for MED13L in this disorder (PMID: 25712080, 25758992) (PVS1). This variant is absent from control populations (https://gnomad.broadinstitute.org/) (PM2) and it has not been reported in individuals with MED13L-related disorders in the databases available for review. Based on the current evidence, this variant is classified as pathogenic for autosomal dominant impaired intellectual development and distinctive facial features with or without cardiac defects.
