Likely Pathogenic for Bernard Soulier syndrome — the classification assigned by ClinGen Platelet Disorders Variant Curation Expert Panel, ClinGen to NM_000407.5(GP1BB):c.96C>A (p.Cys32Ter), citing ClinGen Platelet ACMG Specifications GP1BB V1.0.0. This variant lies in the GP1BB gene (transcript NM_000407.5) at coding-DNA position 96, where C is replaced by A; at the protein level this means converts the codon for cysteine at residue 32 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The c.96C>A (p.Cys32Ter) variant in GP1BB is a nonsense variant that may cause loss of function of the protein, however it is predicted to escape nonsense mediated decay and remove >10% of the protein (PVS1_Strong). At least one patient (Patient BSS4.01 in PMID: 23402648) with this variant had aggregation absent for ristocetin and present for all other agonists as well as less than 10% expression of GPIba and GP9 measured by flow cytometry, which is highly specific for Bernard-Soulier syndrome. Additionally, the patient had excessive mucocutaneous bleeding and macrothrombocytopenia which are consistent with Bernard-Soulier syndrome (PP4). This individual was homozygous for the variant (0.5 points, PM3_Supporting). The variant has been reported to segregate in the proband plus 1 additional homozygous BSS affected family member (1 point, PP1). This variant is absent from gnomAD v4.1.0 (PM2_Supporting). In summary, this variant meets the criteria to be classified as Likely Pathogenic for autosomal recessive Bernard-Soulier syndrome based on the ACMG/AMP criteria applied, as specified by the ClinGen PD VCEP: PVS1_Strong, PP1, PP4, PM2_Supporting and PM3_Supporting (VCEP specifications version 1.1).
