Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_003072.5(SMARCA4):c.1130G>A (p.Arg377His), citing Ambry Variant Classification Scheme 2023. This variant lies in the SMARCA4 gene (transcript NM_003072.5) at coding-DNA position 1130, where G is replaced by A; at the protein level this means replaces arginine at residue 377 with histidine — a missense variant. Submitter rationale: The p.R377H variant (also known as c.1130G>A), located in coding exon 6 of the SMARCA4 gene, results from a G to A substitution at nucleotide position 1130. The arginine at codon 377 is replaced by histidine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 100000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
