Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_005732.4(RAD50):c.3414G>A (p.Met1138Ile), citing Ambry Variant Classification Scheme 2023. This variant lies in the RAD50 gene (transcript NM_005732.4) at coding-DNA position 3414, where G is replaced by A; at the protein level this means replaces methionine at residue 1138 with isoleucine — a missense variant. Submitter rationale: The p.M1138I variant (also known as c.3414G>A), located in coding exon 22 of the RAD50 gene, results from a G to A substitution at nucleotide position 3414. The methionine at codon 1138 is replaced by isoleucine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.01% (grater than 120000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
