Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_004655.4(AXIN2):c.2406-1G>C, citing Ambry Variant Classification Scheme 2023. This variant lies in the AXIN2 gene (transcript NM_004655.4) at the canonical splice acceptor site of the intron immediately before coding-DNA position 2406, where G is replaced by C; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.2406-1G>C intronic variant results from a G to C substitution one nucleotide upstream from coding exon 10 of the AXIN2 gene. This alteration occurs at the 3' terminus of the gene, is not expected to trigger nonsense-mediated mRNA decay, and only impacts the last 5.1% of the protein. This variant was reported in individual(s) with features consistent with Oligodontia-cancer predisposition syndrome (Ambry internal data). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and may result in the creation or strengthening of a novel splice acceptor site. RNA studies have demonstrated that this alteration results in a splice defect; the clinical impact of this abnormal splicing is unknown at this time (Ambry internal data). Based on the available evidence, the clinical significance of this variant remains unclear.
