NM_000179.3(MSH6):c.2529A>G (p.Ile843Met) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the MSH6 gene (transcript NM_000179.3) at coding-DNA position 2529, where A is replaced by G; at the protein level this means replaces isoleucine at residue 843 with methionine — a missense variant. Submitter rationale: The p.I843M variant (also known as c.2529A>G), located in coding exon 4 of the MSH6 gene, results from an A to G substitution at nucleotide position 2529. The isoleucine at codon 843 is replaced by methionine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6490 samples (12980 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 115000 alleles tested) in our clinical cohort. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. In addition, the CoDP in silico tool predicts this alteration will likely impair molecular function, with a score of 0.703 (Terui H et al. J. Biomed. Sci. 2013;20:25). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
