Uncertain significance for Familial melanoma — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000077.5(CDKN2A):c.442G>C (p.Ala148Pro), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CDKN2A gene (transcript NM_000077.5) at coding-DNA position 442, where G is replaced by C; at the protein level this means replaces alanine at residue 148 with proline — a missense variant. Submitter rationale: This sequence change replaces alanine with proline at codon 148 of the CDKN2A (p16INK4a) protein (p.Ala148Pro). The alanine residue is moderately conserved and there is a small physicochemical difference between alanine and proline. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with CDKN2A (p16INK4a)-related disease. ClinVar contains an entry for this variant (Variation ID: 483343). Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The proline amino acid residue is found in multiple mammalian species, suggesting that this missense change does not adversely affect protein function. These predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
