Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_001048174.2(MUTYH):c.1270A>T (p.Thr424Ser), citing Ambry Variant Classification Scheme 2023: The p.T452S variant (also known as c.1354A>T), located in coding exon 14 of the MUTYH gene, results from an A to T substitution at nucleotide position 1354. The threonine at codon 452 is replaced by serine, an amino acid with similar properties. In a massively parallel cell-based functional assay testing 7,8-dihydro-8-oxoguanine:adenine (8OG:A) repair activity, a byproduct of oxidative damage, this variant was reported to be functional (Hemker SL et al. Am J Hum Genet. Published online July 29, 2025. DOI: 10.1016/j.ajhg.2025.07.005). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.
