Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_001042492.3(NF1):c.3975-4G>A, citing Ambry Autosomal Dominant and X-Linked criteria (10/2015). This variant lies in the NF1 gene (transcript NM_001042492.3) at 4 bases into the intron immediately before coding-DNA position 3975, where G is replaced by A. Submitter rationale: The c.3975-4G>A intronic variant results from a G to A substitution 4 nucleotides upstream from coding exon 30 in the NF1 gene. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 110000alleles tested) in our clinical cohort.This nucleotide position is poorly conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is not predicted to have any significant effect on this splice acceptor site; however, direct evidence is unavailable.Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
