Pathogenic for CHD7-related CHARGE syndrome — the classification assigned by Variantyx, Inc. to NM_017780.4(CHD7):c.2605del (p.Leu869fs), citing Variantyx Assertion Criteria 2022. This variant lies in the CHD7 gene (transcript NM_017780.4) at coding-DNA position 2605, deleting one base; at the protein level this means shifts the reading frame starting at leucine residue 869, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This is a frameshift variant in the CHD7 gene (OMIM: 608892). Pathogenic variants in this gene have been associated with autosomal dominant CHARGE syndrome. This variant likely occurred de novo in the current proband; however, the possibility of parental germline mosaicism cannot be excluded (PS2_Moderate). This variant introduces a premature termination codon in exon 8 out of 38 and is expected to result in loss of function, which is a known disease mechanism for CHD7 in this disorder (PMID: 22461308, 25077900) (PVS1). This variant is absent from control populations (https://gnomad.broadinstitute.org/) (PM2) and has not been reported in individuals with CHD7-related disorders in the databases available for review. Based on the current evidence, this variant is classified as pathogenic for autosomal dominant CHARGE syndrome.
