Pathogenic for T-B+ severe combined immunodeficiency due to JAK3 deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000215.4(JAK3):c.1190G>T (p.Gly397Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the JAK3 gene (transcript NM_000215.4) at coding-DNA position 1190, where G is replaced by T; at the protein level this means replaces glycine at residue 397 with valine — a missense variant. Submitter rationale: This sequence change replaces glycine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 397 of the JAK3 protein (p.Gly397Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with JAK3-related conditions (internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt JAK3 protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic.
