NM_000455.5(STK11):c.616G>A (p.Ala206Thr) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the STK11 gene (transcript NM_000455.5) at coding-DNA position 616, where G is replaced by A; at the protein level this means replaces alanine at residue 206 with threonine — a missense variant. Submitter rationale: The p.A206T variant (also known as c.616G>A), located in coding exon 5 of the STK11 gene, results from a G to A substitution at nucleotide position 616. The alanine at codon 206 is replaced by threonine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6013 samples (12026 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 125000 alleles tested) in our clinical cohort. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
