Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_003072.5(SMARCA4):c.4370A>C (p.Asn1457Thr), citing Ambry Variant Classification Scheme 2023. This variant lies in the SMARCA4 gene (transcript NM_003072.5) at coding-DNA position 4370, where A is replaced by C; at the protein level this means replaces asparagine at residue 1457 with threonine — a missense variant. Submitter rationale: The p.N1489T variant (also known as c.4466A>C), located in coding exon 30 of the SMARCA4 gene, results from an A to C substitution at nucleotide position 4466. The asparagine at codon 1489 is replaced by threonine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.003% (greater than 30000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
