Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_005732.4(RAD50):c.3733C>G (p.Leu1245Val), citing Ambry Variant Classification Scheme 2023. This variant lies in the RAD50 gene (transcript NM_005732.4) at coding-DNA position 3733, where C is replaced by G; at the protein level this means replaces leucine at residue 1245 with valine — a missense variant. Submitter rationale: The p.L1245V variant (also known as c.3733C>G), located in coding exon 24 of the RAD50 gene, results from a C to G substitution at nucleotide position 3733. The leucine at codon 1245 is replaced by valine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 175000 alleles tested) in our clinical cohort. This amino acid alignment is not available in most vertebrate species; however, the region around this position is well conserved. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
