Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_005732.4(RAD50):c.688A>C (p.Lys230Gln), citing Ambry Variant Classification Scheme 2023. This variant lies in the RAD50 gene (transcript NM_005732.4) at coding-DNA position 688, where A is replaced by C; at the protein level this means replaces lysine at residue 230 with glutamine — a missense variant. Submitter rationale: The p.K230Q variant (also known as c.688A>C), located in coding exon 5 of the RAD50 gene, results from an A to C substitution at nucleotide position 688. The lysine at codon 230 is replaced by glutamine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 120000 alleles tested) in our clinical cohort. This amino acid position is well conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to abolish the native splice donor site; however, direct evidence is unavailable. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
