Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_002485.5(NBN):c.702+4del, citing Ambry Variant Classification Scheme 2023. This variant lies in the NBN gene (transcript NM_002485.5) at 4 bases into the intron immediately after coding-DNA position 702, deleting one base. Submitter rationale: The c.702+4delA intronic variant, located in intron 6 of the NBN gene, results from a deletion of one nucleotide 4 nucleotides after coding exon 6 of the NBN gene. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6493 samples (12986 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 120000 alleles tested) in our clinical cohort. This nucleotide position is highly conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to weaken the efficiency of the native splice donor site; however, direct evidence is unavailable. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
