Uncertain significance for Frontotemporal dementia and/or amyotrophic lateral sclerosis 4 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_013254.4(TBK1):c.127A>G (p.Ile43Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TBK1 gene (transcript NM_013254.4) at coding-DNA position 127, where A is replaced by G; at the protein level this means replaces isoleucine at residue 43 with valine — a missense variant. Submitter rationale: This sequence change replaces isoleucine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 43 of the TBK1 protein (p.Ile43Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with clinical features of TBK1-related conditions (PMID: 26804609; internal data). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt TBK1 protein function with a negative predictive value of 95%. Experimental studies have shown that this missense change does not substantially affect TBK1 function (PMID: 26804609). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
