This variant lies in the MSH2 gene (transcript NM_000251.3) at coding-DNA position 674, where C is replaced by T; at the protein level this means replaces threonine at residue 225 with isoleucine — a missense variant. Submitter rationale: The MSH2 c.674C>T (p.Thr225Ile) variant has not been reported in individuals with MSH2-related conditions in the published literature. However, it has been observed in an individual with breast cancer (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared)) and in a tumor sample of an individual with thyroid cancer (PMID: 26878173 (2016)). Also, a screening assay based on cell survival in response to 6-thioguanine treatment indicated that this variant has neutral effects on DNA mismatch repair function (PMID: 33357406 (2021)). This variant has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is damaging. Based on the available information, we are unable to determine the clinical significance of this variant.
