Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_004360.5(CDH1):c.793G>A (p.Glu265Lys), citing Ambry Variant Classification Scheme 2023. This variant lies in the CDH1 gene (transcript NM_004360.5) at coding-DNA position 793, where G is replaced by A; at the protein level this means replaces glutamic acid at residue 265 with lysine — a missense variant. Submitter rationale: The p.E265K variant (also known as c.793G>A), located in coding exon 6 of the CDH1 gene, results from a G to A substitution at nucleotide position 793. The glutamic acid at codon 265 is replaced by lysine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6498 samples (12996 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 240000 alleles tested) in our clinical cohort. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
