NM_000059.4(BRCA2):c.7974C>T (p.Tyr2658=) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.7974C>T variant (also known as p.Y2658Y), located in coding exon 16 of the BRCA2 gene, results from a C to T substitution at nucleotide position 7974. This nucleotide substitution does not change the amino acid at codon 2658. This nucleotide position is well conserved in available vertebrate species. In silico splice site analysis predicts that this alteration may weaken the native splice donor site. RNA studies have demonstrated that this alteration results in an incomplete splice defect; the clinical impact of this abnormal splicing is unknown at this time (Ambry internal data). Two saturation genome editing-based studies, including a haploid cell-survival assay and a humanized mouse embryonic stem cell line assay of drug response and survival, demonstrate that this nucleotide substitution is non-functional (Huang H et al. Nature. 2025 Feb;638(8050):528-537; Sahu S et al. Nature. 2025 Feb;638(8050):538-545). Based on the available evidence, the clinical significance of this variant remains unclear.
