Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000059.4(BRCA2):c.9256+3A>C, citing Ambry Variant Classification Scheme 2023: The c.9256+3A>C intronic variant results from an A to C substitution 3 nucleotides after coding exon 23 in the BRCA2 gene. The results from two saturation genome editing-based studies, including a haploid cell-survival assay and a humanized mouse embryonic stem cell line assay of drug response and survival, are discordant for this nucleotide substitution (Huang H et al. Nature, 2025 Feb;638:528-537; Sahu S et al. Nature, 2025 Feb;638:538-545). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration may weaken the native splice donor site; however, direct evidence is insufficient at this time (Ambry internal data). Based on the available evidence, the clinical significance of this variant remains unclear.
