Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000059.4(BRCA2):c.8891G>A (p.Arg2964Lys), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA2 gene (transcript NM_000059.4) at coding-DNA position 8891, where G is replaced by A; at the protein level this means replaces arginine at residue 2964 with lysine — a missense variant. Submitter rationale: The p.R2964K variant (also known as c.8891G>A), located in coding exon 21 of the BRCA2 gene, results from a G to A substitution at nucleotide position 8891. The arginine at codon 2964 is replaced by lysine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.0004% (greater than 225000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
