Uncertain significance for Idiopathic generalized epilepsy; Hyperaldosteronism, familial, type IV — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021098.3(CACNA1H):c.4436G>T (p.Arg1479Leu), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CACNA1H gene (transcript NM_021098.3) at coding-DNA position 4436, where G is replaced by T; at the protein level this means replaces arginine at residue 1479 with leucine — a missense variant. Submitter rationale: This sequence change replaces arginine, which is basic and polar, with leucine, which is neutral and non-polar, at codon 1479 of the CACNA1H protein (p.Arg1479Leu). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with CACNA1H-related conditions. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on CACNA1H protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
