Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000051.4(ATM):c.38G>T (p.Arg13Leu), citing Ambry Variant Classification Scheme 2023. This variant lies in the ATM gene (transcript NM_000051.4) at coding-DNA position 38, where G is replaced by T; at the protein level this means replaces arginine at residue 13 with leucine — a missense variant. Submitter rationale: The p.R13L variant (also known as c.38G>T), located in coding exon 1 of the ATM gene, results from a G to T substitution at nucleotide position 38. The arginine at codon 13 is replaced by leucine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6497 samples (12994 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.0005% (greater than 180000 alleles tested) in our clinical cohort. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be possibly damaging and tolerated by PolyPhen and SIFT in silico analyses, respectively. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
