Uncertain significance for Autosomal dominant nonsyndromic hearing loss 3B; Hidrotic ectodermal dysplasia syndrome; Autosomal recessive nonsyndromic hearing loss 1B; Autosomal recessive nonsyndromic hearing loss 1A — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001110219.3(GJB6):c.377_378insAGGTGAAACCCCGTCTCTACTAAAAATACAAAAAATTAGCCGGGCGCGGTGGCGGGCGCCTGTAGTCCCAGCTNNNNNNNNNNAAAAAAAAAAAAAAAAAAAAAAAAAGCAGAAGGT (p.Val126_Arg127insGlyGluThrProSerLeuLeuLysIleGlnLysIleSerArgAlaArgTrpArgAlaProValValProAlaXaaXaaXaaXaaLysLysLysLysLysLysLysLysGlnLysVal), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GJB6 gene (transcript NM_001110219.3) at coding-DNA position 377 through coding-DNA position 378, inserting AGGTGAAACCCCGTCTCTACTAAAAATACAAAAAATTAGCCGGGCGCGGTGGCGGGCGCCTGTAGTCCCAGCTNNNNNNNNNNAAAAAAAAAAAAAAAAAAAAAAAAAGCAGAAGGT. Submitter rationale: This sequence change inserts a large fragment of DNA, likely a transposable element, in exon 3 of the GJB6 gene (c.377_378ins?), causing a frameshift at codon 127 (p.Arg127fs). The exact size and sequence of the insertion cannot be determined by the current assay. However, the insertion is expected to result in an absent or disrupted protein product. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with GJB6-related conditions. Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. Retrotransposon insertions including LINE1 (L1), Alu, and SVA (SINE-VNTR-Alu) have been reported to disrupt protein function (PMID: 19763152, 20307669, 22406018). However the effect of this particular variant is unknown. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
