Pathogenic for Dilated cardiomyopathy 1HH; Myofibrillar myopathy 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004281.4(BAG3):c.863_867del (p.His288fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the BAG3 gene (transcript NM_004281.4) at coding-DNA position 863 through coding-DNA position 867, deleting 5 bases; at the protein level this means shifts the reading frame starting at histidine residue 288, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.His288Profs*29) in the BAG3 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 288 amino acid(s) of the BAG3 protein. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with BAG3-related conditions. This variant disrupts a region of the BAG3 protein in which other variant(s) (p.Arg309*) have been determined to be pathogenic (PMID: 25448463). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
