This variant lies in the IRF6 gene (transcript NM_006147.4) at coding-DNA position 1181, where T is replaced by A; at the protein level this means replaces valine at residue 394 with aspartic acid — a missense variant. Submitter rationale: This sequence change replaces valine, which is neutral and non-polar, with aspartic acid, which is acidic and polar, at codon 394 of the IRF6 protein (p.Val394Asp). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of Van der Woude syndrome (internal data). In at least one individual the variant was observed to be de novo. An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively.
