Pathogenic for Dilated cardiomyopathy 1HH; Myofibrillar myopathy 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004281.4(BAG3):c.1366G>C (p.Glu456Gln), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glutamic acid, which is acidic and polar, with glutamine, which is neutral and polar, at codon 456 of the BAG3 protein (p.Glu456Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of dilated cardiomyopathy (internal data). In at least one individual the variant was observed to be de novo. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt BAG3 protein function with a positive predictive value of 80%. For these reasons, this variant has been classified as Pathogenic.
