Uncertain significance for Primary familial hypertrophic cardiomyopathy; Dilated cardiomyopathy 1AA — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001103.4(ACTN2):c.682A>C (p.Met228Leu), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces methionine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 228 of the ACTN2 protein (p.Met228Leu). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with ACTN2-related conditions. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt ACTN2 protein function with a positive predictive value of 80%. This variant disrupts the p.Met228 amino acid residue in ACTN2. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 25173926, 37271035). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
