Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_177438.3(DICER1):c.2743A>G (p.Thr915Ala), citing Ambry Variant Classification Scheme 2023. This variant lies in the DICER1 gene (transcript NM_177438.3) at coding-DNA position 2743, where A is replaced by G; at the protein level this means replaces threonine at residue 915 with alanine — a missense variant. Submitter rationale: The p.T915A variant (also known as c.2743A>G), located in coding exon 16 of the DICER1 gene, results from an A to G substitution at nucleotide position 2743. The threonine at codon 915 is replaced by alanine, an amino acid with similar properties. This alteration was identified in 1/1358 non-cancer control individuals and in 0/57 cases, in a study looking at cancer predisposition mutations in patients with cutaneous melanoma and a history of at least two additional non-cutaneous melanoma primary cancers (Pritchard AL et al. PLoS One, 2018 Apr;13:e0194098). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
