Pathogenic for Tumor predisposition syndrome 3 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_015450.3(POT1):c.1686+4A>G, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the POT1 gene (transcript NM_015450.3) at 4 bases into the intron immediately after coding-DNA position 1686, where A is replaced by G. Submitter rationale: This sequence change falls in intron 17 of the POT1 gene. It does not directly change the encoded amino acid sequence of the POT1 protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or altered protein product. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with POT1-related conditions. ClinVar contains an entry for this variant (Variation ID: 475056). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant results in activation of a cryptic splice site, and produces a non-functional protein and/or introduces a premature termination codon (internal data). For these reasons, this variant has been classified as Pathogenic.
