Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000162.5(GCK):c.271G>T (p.Val91Leu), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 91 of the GCK protein (p.Val91Leu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with clinical features of autosomal dominant familial hyperinsulinemic hypoglycemia (PMID: 20375417, 28247534, 37725835). It has also been observed to segregate with disease in related individuals. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt GCK protein function with a negative predictive value of 80%. Experimental studies have shown that this missense change affects GCK function (PMID: 20375417, 21831042). For these reasons, this variant has been classified as Pathogenic.
