Uncertain significance for Actin accumulation myopathy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001100.4(ACTA1):c.203C>T (p.Thr68Ile), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces threonine, which is neutral and polar, with isoleucine, which is neutral and non-polar, at codon 68 of the ACTA1 protein (p.Thr68Ile). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with autosomal dominant nemaline myopathy or clinical features of this condition (PMID: 15198992; internal data). In at least one individual the variant was observed to be de novo. This variant is also known as T66I. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt ACTA1 protein function with a negative predictive value of 80%. Experimental studies are conflicting or provide insufficient evidence to determine the effect of this variant on ACTA1 function (PMID: 15198992). This variant disrupts the p.Thr68 amino acid residue in ACTA1. Other variant(s) that disrupt this residue have been observed in individuals with ACTA1-related conditions (PMID: 35810298), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
