Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001024630.4(RUNX2):c.569G>T (p.Arg190Leu), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with leucine, which is neutral and non-polar, at codon 190 of the RUNX2 protein (p.Arg190Leu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with cleidocranial dysplasia (internal data). In at least one individual the variant was observed to be de novo. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt RUNX2 protein function with a positive predictive value of 80%. This variant disrupts the p.Arg190 amino acid residue in RUNX2. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 11768584, 12196916, 16221346). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
