Likely pathogenic for PTEN hamartoma tumor syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000314.8(PTEN):c.95T>G (p.Ile32Ser), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the PTEN gene (transcript NM_000314.8) at coding-DNA position 95, where T is replaced by G; at the protein level this means replaces isoleucine at residue 32 with serine — a missense variant. Submitter rationale: This sequence change replaces isoleucine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 32 of the PTEN protein (p.Ile32Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with PTEN-related conditions (PMID: 39358013). In at least one individual the variant was observed to be de novo. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on PTEN protein function. Experimental studies have shown that this missense change affects PTEN function (PMID: 29706350). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
