Likely pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_004656.4(BAP1):c.1984-1G>A, citing Ambry Variant Classification Scheme 2023: The c.1984-1G>A intronic variant results from a G to A substitution one nucleotide upstream from coding exon 16 of the BAP1 gene. Variants that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site. A resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; although, direct evidence is unavailable. However, the region predicted to be impacted is critical for protein function (Ambry internal data). This variant was non-functional in a high throughput genome editing haploid cell survival functional assay (Waters AJ et al. Nat Genet, 2024 Jul;56:1434-1445). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. Based on the majority of available evidence to date, this variant is likely to be pathogenic.
