Uncertain significance for Pheochromocytoma/paraganglioma syndrome 5; Mitochondrial complex II deficiency, nuclear type 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004168.4(SDHA):c.1863C>G (p.His621Gln), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SDHA gene (transcript NM_004168.4) at coding-DNA position 1863, where C is replaced by G; at the protein level this means replaces histidine at residue 621 with glutamine — a missense variant. Submitter rationale: This variant has not been reported in the literature in individuals with SDHA-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C15"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant is not present in population databases (ExAC no frequency). This sequence change replaces histidine with glutamine at codon 621 of the SDHA protein (p.His621Gln). The histidine residue is highly conserved and there is a small physicochemical difference between histidine and glutamine.
