Uncertain significance for Primary ciliary dyskinesia — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000328.3(RPGR):c.2319dup (p.Ser774fs), citing Invitae Variant Classification Sherloc (09022015): This sequence change creates a premature translational stop signal (p.Ser774Ilefs*11) in the RPGR gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 42 amino acid(s) of the RPGR protein. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with RPGR-related conditions. Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This variant disrupts the C-terminus of the RPGR protein. Other variant(s) that disrupt this region (p.Ile775Argfs*9, p.Asp787Argfs*10) have been observed in individuals with RPGR-related conditions (PMID: 33247286, 36819107). This suggests that this may be a clinically significant region of the protein. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
