Uncertain significance for Autosomal recessive limb-girdle muscular dystrophy type 2Q; Epidermolysis bullosa simplex, Ogna type; Epidermolysis bullosa simplex with nail dystrophy; Epidermolysis bullosa simplex 5C, with pyloric atresia; Epidermolysis bullosa simplex 5B, with muscular dystrophy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_201384.3(PLEC):c.8329T>C (p.Tyr2777His), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces tyrosine with histidine at codon 2804 of the PLEC protein (p.Tyr2804His). The tyrosine residue is moderately conserved and there is a moderate physicochemical difference between tyrosine and histidine. This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature in individuals with a PLEC-related disease. In summary, this variant is a novel missense change with uncertain impact on protein function. It has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies.
