Pathogenic for Short-rib thoracic dysplasia 11 with or without polydactyly — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_052844.4(DYNC2I2):c.1280_1283del (p.Pro427fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the DYNC2I2 gene (transcript NM_052844.4) at coding-DNA position 1280 through coding-DNA position 1283, deleting 4 bases; at the protein level this means shifts the reading frame starting at proline residue 427, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Pro427Argfs*48) in the WDR34 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 110 amino acid(s) of the WDR34 protein. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with WDR34-related conditions. This variant disrupts a region of the WDR34 protein in which other variant(s) (p.Thr514Argfs*11) have been determined to be pathogenic (PMID: 24183451). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
