Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_018684.4(ZC4H2):c.602C>T (p.Pro201Leu), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 201 of the ZC4H2 protein (p.Pro201Leu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with ZC4H2-related conditions (PMID: 36250278). In at least one individual the variant was observed to be de novo. An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. This variant disrupts the p.Pro201 amino acid residue in ZC4H2. Other variant(s) that disrupt this residue have been observed in individuals with ZC4H2-related conditions (PMID: 23623388, 31206972, 36250278), which suggests that this may be a clinically significant amino acid residue. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
