This variant lies in the TTN gene (transcript NM_001267550.2) at coding-DNA position 90420, duplicating one base; at the protein level this means shifts the reading frame starting at valine residue 30141, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: Variant summary: TTN NM_133378:c.82716dupA (p.Val27573SerfsX12) (also known as NM_001267550:c.90420dupA (p.Val30141SerfsX12)) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Loss of function variants in all TTN bands are strongly associated with a spectrum of autosomal recessive titinopathies when exon expression (proportion spliced in, PSI, 1=complete expression) in skeletal muscle is >0.1 (PMID: 36977548, 39198997, 29598826, 32778822, 29691892, 33449170, 36977548, internal data). To our knowledge, no occurrence of c.82716dupA in individuals affected with TTN-related conditions and no experimental evidence demonstrating its impact on protein function have been reported. Based on the evidence outlined above, the variant was classified as likely pathogenic for both autosomal dominant and autosomal recessive TTN-related conditions.
