Uncertain Significance — the classification assigned by ARUP Laboratories, Molecular Genetics and Genomics, ARUP Laboratories to NM_000517.6(HBA2):c.121A>G (p.Lys41Glu), citing ARUP Molecular Germline Variant Investigation Process 2024: The Hb Kariya variant (HBA2: c.121A>G; p.Lys41Glu, also known as Lys40Glu when numbered from the mature protein, rs281864827, HbVar ID:52) is reported in the literature in one heterozygous individual who did not have clinical symptoms (Harano 1983, HbVar database and references therein). This variant is reported as a slightly unstable hemoglobin variant with increased oxygen affinity (Imai 1989, HbVar database and references therein). This variant is absent from the Genome Aggregation Database (v2.1.1), indicating it is not a common polymorphism. Another variant at this codon (Hb Linwood, HbVar ID: 1255, c.121A>C; p.Lys41Gln) have been reported in a mother and baby whose hemoglobin was in the high range of normal (HbVar database and references there in). Additionally, another variant in the homologous gene HBA2 (Hb Saratoga springs, HbVar ID:1117, c.123G>C, p.Lys41Asn) has been reported in individuals with moderate erythrocytosis (see HbVar database and references therein). Computational analyses predict that this variant is deleterious (REVEL: 0.909). Due to limited information, the clinical significance of this variant is uncertain at this time.
