Uncertain significance for Lethal congenital glycogen storage disease of heart — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_016203.4(PRKAG2):c.1337T>G (p.Ile446Ser), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces isoleucine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 446 of the PRKAG2 protein (p.Ile446Ser). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with PRKAG2-related conditions. This missense change has been observed to co-occur in individuals with a different variant in PRKAG2 that has been determined to be pathogenic (Invitae), but the significance of this finding is unclear. ClinVar contains an entry for this variant (Variation ID: 465339). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on PRKAG2 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
