Uncertain significance for Pyridoxine-dependent epilepsy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001182.5(ALDH7A1):c.952G>C (p.Ala318Pro), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces alanine with proline at codon 318 of the ALDH7A1 protein (p.Ala318Pro). The alanine residue is moderately conserved and there is a small physicochemical difference between alanine and proline. This variant is not present in population databases (ExAC no frequency). This variant has been reported in combination with another ALDH7A1 variant in an individual affected with pyridoxine-dependent epilepsy (PMID: 24664145). In summary, this variant is a rare missense change with uncertain impact on protein function. While it is absent from the population and reported in an affected individual, the available evidence is currently insufficient to determine its role in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies.
