Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_001005242.3(PKP2):c.364C>T (p.Arg122Cys), citing Ambry Variant Classification Scheme 2023. This variant lies in the PKP2 gene (transcript NM_001005242.3) at coding-DNA position 364, where C is replaced by T; at the protein level this means replaces arginine at residue 122 with cysteine — a missense variant. Submitter rationale: The p.R122C variant (also known as c.364C>T), located in coding exon 3 of the PKP2 gene, results from a C to T substitution at nucleotide position 364. The arginine at codon 122 is replaced by cysteine, an amino acid with highly dissimilar properties. This variant was detected in an arrhythmia genetic testing cohort; however, clinical details were limited (van Lint FHM et al. Neth Heart J, 2019 Jun;27:304-309). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.
