This variant lies in the BRCA1 gene (transcript NM_007294.4) at the canonical splice donor site of the intron immediately after coding-DNA position 5277, where G is replaced by T; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.5277+1G>T intronic pathogenic mutation results from a G to T substitution one nucleotide after coding exon 18 of the BRCA1 gene. Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. The resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; however, direct evidence is unavailable. The exact functional effect of the altered amino acid sequence is unknown, but the impacted region is critical for protein function (Ambry internal data). One functional study found that this nucleotide substitution is non-functional in a high-throughput, genome editing, haploid cell survival assay (Findlay GM et al. Nature, 2018 10;562:217-222). This nucleotide position is highly conserved in available vertebrate species. Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation.
