Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000059.4(BRCA2):c.1400A>G (p.Lys467Arg), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA2 gene (transcript NM_000059.4) at coding-DNA position 1400, where A is replaced by G; at the protein level this means replaces lysine at residue 467 with arginine — a missense variant. Submitter rationale: The p.K467R variant (also known as c.1400A>G), located in coding exon 9 of the BRCA2 gene, results from an A to G substitution at nucleotide position 1400. The lysine at codon 467 is replaced by arginine, an amino acid with highly similar properties. This alteration was detected in a cohort of 70 Dutch breast and/or ovarian cancer patients (G&oacute;mez-Garc&iacute;a EB et al. J Clin Oncol, 2005 Apr;23:2185-90), and in a cohort of 133 unselected Burkina Faso breast cancer patients (Ouedraogo SY et al. Mol Genet Genomics, 2022 Sep;297:1257-1268). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
